Please complete the forms prior to your arrival at our office and we will serve you promptly! For your security, you cannot send them electronically.

If you cannot print the forms, call Alex and she will mail or fax them to you if you would like.

If you cannot complete the forms prior to your first visit, plan to arrive at our office 20 minutes before your scheduled appointment in order to complete the paperwork.
